Center Policy
The following are the current office policies of Sanctuary Treatment Center: It is subject to change without prior notice.
Appointment

Appointments can be booked online or in person, and over the phone by calling (480)-256-1680.

When booking your appointment, please let us know the reason you wish to see the doctor. This will help us prepare for your visit and provide adequate and reasonable time for you with the doctor.

There is 24 hours’ cancellation notice requirement if you are not able to make your appointment. The Center will charge for all missed appointments. This charge must be paid prior to seeing another doctor at our Center. If you have more than two consecutive no-show appointments, reschedules, or cancellations, you run the risk of your services being discontinued at Sanctuary Treatment Center.

As soon as we receive your request to transfer your medical records to a new Psychiatric/Mental Health provider, we will cancel your upcoming appointments with or w/o notice. If you would like to make a new appointment, you need to contact us again.

Walk-in patients are not accepted.
Test or Lab Results

We expect patients to come back to our office for test results. The doctor will let you know when to come back when you see him/her at the end of your current visit.

The clinic may call and inform you when lab results are ready. If you have not heard back, please give the clinic a call at (480)-256-1680.

Test results are not provided over the phone for privacy reasons.
Prescriptions and Refills

Please call and check with your pharmacy to verify the pharmacy has received the script(s) and Do NOT rely on Pharmacy App information as the information is oftentimes unreliable.

Prescriptions and refills of DEA controlled medication can NOT be sent without evaluation with our provider(s).
Forms and Medical Records

Standard forms are available at the front desk and we can send them to you by email.

Other forms (FMLA etc) are expected to be provided by the requesting party. Providers will not be able to complete these forms until patients are fully established with the provider for at least three months.

There is a $40 fee for medical records. This fee is waived if records are requested by another physician who is providing continuing care. Please have the provider fax a signed medical release form requesting your records to us at 1-888-609-4966. Records requests can take up to 30 days to be processed.
Co-Pays and Coinsurance

All co-pays are due at the time of check-in. Some AHCCCS (Arizona Health Care Cost Containment System) plans have implemented mandatory and non-mandatory co-pays. Please check with your insurance provider, or Arizona Department of Economic Security (DES), to determine if you will have a co-pay.

We will request credit card/debit card information for Tele-Health patients and these information is required on file as there is no Check-In process for patients, who request Tele-Health Services. The billing department will automatically charge patients’ credit card/debit card for the copay and coinsurance before each virtual visit. It is your responsibility to keep us have most updated card information. If the credit card/debit card is declined, an invoice will be sent by an email. The Zoom meeting link will be sent only after the copay and coinsurance are fully collected.

It is the patient’s responsibility to provide us with up-to-date phone numbers, home addresses, emails, and emergency contact information. Updates can be done via sending email to info@sanctuarytreatmentcenter.net or by calling us with the information at (480) 256- 1680.

Please inform the receptionist upon arrival at the Center if your contact information has changed.
Controlled Medications

For your safety, proper use, and to minimize the risks of overuse and adverse side effects, we follows special prescribing rules for narcotic medications.

It is our policy not to prescribe controlled substances to patients who are not known to our center.

Refills are not provided at times earlier than those decided upon at the last visit.

We ask that patients receiving these medications receive their prescriptions from one particular doctor and fill those prescriptions at one particular pharmacy. Irregularities may result in the end of your prescription.
Behavior
-
The behaviors hereby listed below are subject to discharge from the practice without prior notice. We reserve the right to refuse service if any of the following behavior occurs.

Non-payment, failure to make payment or default on payment.

Noncompliance with treatment including irregular follow-up, multiple no-shows, and multiple cancellations without legitimate reason.

Any abusive, aggressive or assaultive language or/and behavior.

Non-disclosure of any critical medical/psychiatric information
